Service Coordination

Navigating the wide range of supports and services available to people with developmental disabilities can be a difficult task without the guidance of an experienced, well-trained Medicaid Service Coordinator. Service Coordinators facilitate access to needed services and benefits and serve as advocates, who ensure supports are moving each individual toward the attainment of life goals.

In July 2018, this service is scheduled to transfer into a more comprehensive Care Coordination service, facilitated by the New York State Office for People With Developmental Disabilities. Regional Care Coordination Organizations will be selected by each individual to provide their care coordination moving foward.
